Shortfall Fund announcement for the contribution period July 1, 2020 to Dec. 31, 2020
To All Freelance Orchestra Musicians:
If you are a ROSTERED MEMBER of one or more freelance orchestra(s) with a negotiated health benefit shortfall fund provision in your Collective Bargaining Agreement (see the list at the very end of this article), please read carefully:
The application form will go live at 9:30 am on January 19, 2021. You may access the application here: https://www.local802afm.org/contracts/shortfall-funds . We cannot process requests via direct e-mail. Although most contributions will be credited to musicians’ accounts by Jan. 19, there may be omissions or delays beyond our control. Please make a best estimate of how much you will need to reach either the $500 threshold for Plan B or $2000 for Plan A. We will confirm the amount with the fund before we send the shortfall request to the orchestra(s).
Please remember that each request is limited to the maximum amount (depending on the orchestra- please refer to the chart) needed to get to Plan A (major medical) or to Plan B (vision and dental only) You may not utilize the shortfall fund to move from below the contribution level for Plan B ($500) to Plan A.
Requests are made on a first come, first served basis. Local 802 and the Local 802 Health Benefits Fund are not responsible for fulfilling requests made after the shortfall funds have been exhausted or for requests that it does not receive properly. Please request funds only if you intend on becoming a plan participant by paying the participant premium.
